Welcome to LJ! You save the icon to your hard drive and then, if you're logged into your LJ account, go here:

http://www.livejournal.com/editpics.bml

to upload. You click on Browse, and navigate to the folder where you saved the icon.

One word of warning: some icon makers on LJ are picky about being credited in the Comment Field for the icon; they want you to list their username there. I'm not particular but many are. Save yourself some heartache by taking note of the "rules" in an icon post and credit those who demand it. I do it regardless of whether somebody demands it or not as it's handy to know who made what. It's the community norm here on LJ to respect a maker's reasonable requests and woe unto she who inadvertently crosses that line.
